Abstract
In this article, the authors discuss the growing crisis of opioid access barriers for people with serious illness, who commonly require these medications as a first-line pain treatment. The authors report the results of a needs assessment survey they conducted among clinicians in the Department of Geriatrics and Palliative Medicine at Mount Sinai Health System in New York City regarding perceived challenges to opioid access for patients with serious illness. The survey, corroborated by incident reports, identified multiple barriers related to providers, pharmacies, and insurers. The surveyed clinicians reported that barriers to opioid access resulted in patients not receiving optimal pain relief, higher rates of hospital readmission, poorer quality of life, and reduced job satisfaction for clinicians. The article outlines a broader framework for local and national action to address this pressing problem.
