Introduction
Semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist, has produced substantial weight loss in clinical trials where patient adherence exceeded 85%.1 In routine practice, adherence is considerably lower,2 and benefits of GLP-1 receptor agonists depend on sustained medication use.3 Studies among patients receiving GLP-1 receptor agonists for obesity have documented substantial treatment discontinuation and identified financial barriers to treatment continuation.4,5 However, less is known about how patient cost sharing relates to adherence, which reflects the consistency of medication use over time and may be affected by financial barriers even among patients who continue treatment. We examined the association between patient out-of-pocket costs and semaglutide adherence among commercially insured US adults with obesity, for whom cost sharing can be substantial.
