ワシントン大学セントルイス(WashU)の研究チームは、がん経験者は、がんの既往歴がない成人と比べて身体活動量が少ないことを明らかにした。米国の成人200万人超を対象に2018~2024年の全国代表データを分析した結果、両群とも調査期間中に身体活動は増加したものの、がん経験者の活動量は一貫して低かった。研究者は、がん診断後の身体活動には、がん関連疲労の軽減、生活の質、身体機能、精神的健康などへの利益が報告されていると指摘する。一方、がん経験者は健康状態、身体能力、治療の影響、症状や副作用が多様であるため、一律の運動目標を設定するのではなく、個人に合わせた活動を推奨すべきだとしている。医療従事者や介護者による継続的な助言・社会的支援も、身体活動の開始・継続を支える重要な要素とされる。

ABSTRACT

Introduction
: The decline in U.S. cancer deaths from early detection and treatment advancements has markedly increased the number of cancer survivors. Regular physical activity (PA) after cancer diagnosis has been associated with numerous health benefits, including reduction in cancer-related fatigue, quality of life, physical fitness and functioning, and improved mental health. This study aims to explore PA trends, differences, and meeting PA guidelines in adults with vs. without a cancer history using national data.

Methods
The study used Behavioral Risk Factor and Surveillance data from 2018-2024 to estimate survey-weighted annual PA prevalences among adults with and without a cancer history. Trends were assessed using log-linear regression. Aerobic guideline adherence and cancer-type-specific estimates were also examined for prostate, breast, colon, and other cancers.

Results
Among over two million adults, PA prevalence increased significantly among adults with a cancer history (67.1% to 71.6%; annual percentage change (APC) = 1.05%; 95% CI, 0.6% – 1.5%) but not those without a cancer history (76.3% to 78.3%; APC = 0.47%; 95% CI, -0.19% – 1.14%). Cancer history was associated with a 3.8 percentage-point lower adjusted prevalence of PA. Aerobic PA guideline adherence was similar by cancer history.

Conclusions
: Cancer survivors had a lower PA prevalence across the study time-period than those with no cancer history. Similar rates of meeting the PA guidelines for aerobic activity were observed. The higher prevalence of no PA among cancer survivors presents an opportunity to promote PA through targeted interventions for healthcare provider counseling and cancer-specific training by exercise specialists.
