In the Medicare Advantage (MA) program, private plans receive capitated payments to provide benefits to Medicare beneficiaries.1 Most beneficiaries have access to more than 40 plans in their county.2 To help navigate these choices, a robust ecosystem of brokers has emerged.3 Some brokers are independent while others work exclusively for a single insurer. MA plans directly pay brokers an initial commission for each new enrollee they bring in and a renewal fee for beneficiaries who remain in the plan. Brokers may fill an important gap by helping beneficiaries make choices. However, brokers are not a fiduciary of the beneficiary and may enroll beneficiaries in plans not based on their needs but based on which compensate them the most through commissions and bonus payments meeting enrollment targets. Prior survey-based research suggests that about one-third of beneficiaries used brokers annually, but limited quantitative data exist on the use of brokers nationally.3
