障害のある医療者「にもできる」ではなく 「にしかできない」貢献 ――脳性まひを生きる医師の当事者研究から発見された「障害の専門性」――

従来の医学における障害の「欠損モデル」を見直し、障害のある人が生活や社会への適応過程で獲得する独自の知識・技能を「障害の専門性(Disability Expertise)」として捉え直した研究。脳性まひを持つ小児科医の臨床経験を分析し、障害の専門性が医療の質を高める3つの仕組みを明らかにした。①標準手順に固執せず、医療行為の本質的目的から柔軟な方法を考案する力、②標準から外れた複雑な状況を新たな価値創造の機会と捉える視点、③相互依存の重要性を理解し、チーム内の信頼と協働を促す力である。さらに、障害のある医療者を増やすこと、標準化と個別化のバランス、多様な専門家が補完し合う組織文化の重要性を示した。障害を「弱み」ではなく医療を改善する資源として位置づけ、包摂的で持続可能な医療体制の構築につながる成果である。

SK, the lead author of this article, was born with cerebral palsy, is supported by personal assistants for daily living activities, and uses a power wheelchair. Thanks to his family’s advocacy and steadfast belief in his capacities, he was educationally mainstreamed with peers, which was atypical at the time in Japan. His application to medical school sparked a debate about whether he could become a doctor. A faculty member visited SK’s apartment to assess him and met the group of people who were part of his care community. After observing SK’s many relationships, the faculty member advocated for his admission.
