心臓カテーテル治療などの放射線を用いる手技では、患者から発生する散乱線により医療従事者、とくに頭部・眼・手が被ばくすることが課題となっている。東北大学の研究グループは、患者上に置く従来の防護ドレープに、取り外し・角度調整可能な放射線遮蔽壁を組み合わせた「壁付ドレープ」を開発した。総重量は938gと軽量で、心臓カテーテル治療を想定した7方向から防護性能を評価した結果、壁を追加することで術者の頭部や手の放射線量を約70~80%低減できた。従来型防護具の重量による患者負担を抑えながら、医療従事者の職業被ばくを低減できる可能性がある。今後、実際の医療現場への導入が期待される。

Abstract

Occupational radiation exposure remains a concern during interventional radiology (IR) procedures. Patient-mounted shielding drapes can reduce exposure, but conventional drapes are heavy, and performance of lightweight drapes with shielding walls has not been investigated. This study developed a lightweight X-ray shielding drape with a detachable shielding wall and evaluated its radiation shielding performance under different projection angles and operator positions. A phantom study was performed using an angiography system under seven projection angles during percutaneous coronary intervention. Ambient dose equivalent was measured at four operator locations: the torso, eye lens, forward-leaning eye lens, and hand. The device consisted of a shielding drape and a detachable shielding wall, both with a lead equivalence of 0.125 mmPb and a combined weight of 938 g. Dose reduction was evaluated under four conditions: no protection, drape alone, drape with a vertical shielding wall, and drape with a forward-tilted shielding wall. The shielding wall provided protection compared with the drape alone. Dose reductions of 70–80% were achieved at the forward-leaning eye lens and hand positions. The effect varied with projection angle and was limited at the eye lens position for some LAO and CAU projections. No significant difference was observed between the vertical and forward-tilted configurations. These findings indicate that the lightweight shielding drape can provide radiation protection despite its low lead equivalence. Its detachable shielding wall may provide complementary protection at positions that are difficult to cover with conventional devices. The lightweight design may reduce the physical burden on patients during IR procedures.
